APPLICATION FOR A WEST VIRGINIA NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM 
WATER POLLUTION CONTROL PERMIT

The following has applied for a WV NPDES Water Pollution Control Permit for 
this facility or activity:

Activity:	
operate and maintain a treatment and disposal system and best management 
practices for the direct discharge of untreated cooling water blowdown and 
treated sanitary wastewater via Outlet 001 into the Big Sandy River at 
approximate Mile Point 3.56.  Also, to operate and maintain a disposal system 
and best management practices for the direct discharge of untreated storm water 
via Outlet 002 into an unnamed tributary of the Big Sandy River, and untreated 
storm water and firewater tank overflow via Outlet 003 into the Big Sandy River 
at approximate Mile Points 3.57.  

Also to operate and maintain a disposal system and best management practices 
for the direct discharge of untreated storm water via Outlet 004, 005 and 006 
into the Big Sandy River at approximate Mile Point 3.57.

Also to acquire, construct, install operate, and maintain a new sanitary 
treatment package plant.  The existing package plant will be replaced with a 
PURESTREAM Model PT-3.  This unit consists of the same operating parameters and 
specifications as the existing package plant.  

Tier 1 protection is afforded at Outlet 001 for the uses specified in 47 CSR 2, 
Section 6.

	On the basis of review of the application, the "Water Pollution 
Control Act (Chapter 22, Article 11-8(a))," and the "West Virginia Legislative 
Rules," the State of West Virginia will act on the above application.

	Any interested person may submit written comments on the draft permit 
and may request a public hearing by addressing such to the Director of the 
Division of Water and Waste Management within 30 days of the date of the public 
notice.  Such comments or requests should be addressed to:

	The public comment period begins February 05, 2020 and ends March 06, 
2020.

	Comments received within this period will be considered prior to 
acting on the permit application.  Correspondence should include the name, 
address and the telephone number of the writer and a concise statement of the 
nature of the issues raised.  The Director shall hold a public hearing whenever 
a finding is made, on the basis of requests, that there is a significant degree 
of public interest on issues relevant to the Draft Permit(s).  Interested 
persons may contact the public information office to obtain further information.

	The application, draft permit and any required fact sheet may be 
inspected, by appointment, at the Division of Water and Waste Management Public 
Information Office, at 601 57th Street SE, Charleston, WV 25304-2345, between 
8:00 a.m. and 4:00 p.m. on business days.
